New Patients

New Patient Registration

All newly registered patients will be asked to complete a questionnaire and have a consultation with the doctor or nurse for a New Patient Screening. This includes a height, weight and blood pressure check and allows us to summarise your medical history whist awaiting your medical records from your previous GP. Details of past illness, vaccinations and smear history, are also taken where applicable. 

You will be asked to bring a sample of urine for testing. Failure to attend for this appointment may lead to refusal of registration. Medical treatment is available from the date of registration.

Catchment Area

We are current registering patients from the following areas:

  • Heaton
  • Lostock
  • Ladybridge
  • Smithills

Our extended area for patients who may move out of our normal catchment areas are:

  • Horwich
  • Halliwell
  • Astley Bridge

Catchment Area

Patients living within this area are welcome to register with us.

We will require proof of address e.g. Utility bill or driving license and proof of ID e.g. Passport
